When do you need a model release form for photography?

If I took a picture of a group of strangers, I need a permit Model of each to put my photo? Furthermore, it is necessary to obtain a model release, if a person is not recognizable (back of head, silhouette etc)? And what is the best way to keep track of who's who in the model releases? If you have five people in a photo, how to know what version belongs to each person?

Who decides that a person is unrecognizable always best to get a statement signed to play it safe. Photos taken at places not require a public statement in most cases is public, but if you are taking photographs of specific individuals and provide guidance on how to position themselves (to pose for you) receive a signed statement. If in doubt get a statement that would be a shame if you have a great movie, but could not use it for legal reasons.
